Find your location and position title in Appendix A of the Conflict of Interest Code. Appendix A indicates the applicable disclosure category number(s) by position title.
Review the explanation of your disclosure category number(s) contained in Appendix B of the Conflict of Interest Code. The disclosure category(ies) will explain the specific types of economic interests you must disclose and indicate which Schedules in Form 700 you must complete.
Complete the cover page and all applicable schedules of the Form 700.
Sign and date the cover page of the Form 700 in blue ink.
Send the completed Form 700 with the original signature to: Coordinator Ross Smith, 1111 Franklin Street, 8th Floor, Oakland, CA 94607-5200. Annual forms are due April 1, 2008
